About Lincolnshire Community Health Services NHS Trust

Lincolnshire Community Health Services NHS Trust is the primary community healthcare provider in Lincolnshire. The trust delivers a broad range of community nursing, therapy, urgent care, reablement, palliative care, public health, children’s health and social care services.

By providing community-based services aimed at preventing health problems from getting worse, reducing the need for people to go into hospital. We are working closely with other health and social care services to support a shift from care in acute hospitals, into more joined-up care in the community, closer to home.
